“Special Focus Facility” Nursing Homes Watchlist

List Updated: September 15, 2016

The average nursing home has between six and seven deficiencies during an inspection, but most of these facilities correct problems within an appropriate amount of time. On the other hand, a small percentage of nursing home facilities demonstrate:

  • A pattern of serious problems over an extended period of time.
  • Serious problems, compared to other facilities. This includes deficiencies leading to injury or harm.
  • Twice the average number of deficiencies.

According to CMS, the Centers for Medicare & Medicaid Services, it is common for SFFs to exhibit compliance one month, only to show new deficiencies by the next inspection. Because of this, the CMS created the SFF list. This list includes facilities that demonstrate patterns of serious problems and, according to CMS, have an underlying systematic problem that creates repetitive cycles of serious deficiencies.

Each facility on the SFF list falls into one of five categories: new additions, not improved, improving, recently graduated, or “no longer in Medicare and Medicaid.” Below, you will find an updated list of SFF facilities organized by table.

Facilities in Table E were recently terminated from participating in Medicare and Medicaid. Very few nursing homes remain open after CMS terminates them from the Medicare program.

Additional Information About SFF Nursing Homes

CMS will only allow a nursing home to graduate from the SFF program when it exemplifies “significant improvements in quality of care.” Additionally, these improvements cannot be temporary; they must exist for a substantial period of time.

If CMS determines that the facility should be discontinued from Medicare, the nursing home will no longer receive funding from Medicare and Medicaid. This usually means that the nursing home will close, and residents will be transferred to new facilities.
